Insurance

We will work with any insurance plan including Medicare and Arizona Medicaid (AHCCCS).

Payment is due prior to delivery of your prosthesis (not including AHCCCS programs). We accept credit cards for your convenience. Most insurance companies reimburse patients for the partial or total cost of a new or replacement prosthesis. We will assist patients in submitting requests to their insurance provider for pre-approval and reimbursement.

All insurance companies require that a physician authorize the medical necessity for an artificial eye whether it is a new or replacement prosthesis. We advise patients to request this prescription from their ophthalmologist:

For artificial eye patients – “Fit with custom conformer and artificial eye (left, right, or both)”

For scleral shell patients – “Fit with custom conformer and scleral shell (left, right, or both)”

For pediatric patient expansion conformers – “Fit with series of custom expansion conformers and artificial eye or scleral shell (left, right, or both)”
